ReachOut Health Hub Program

The ReachOut Health Hub Program delivers accessible, multidisciplinary healthcare support to individuals and families within the community. Designed as a flexible outreach model, the program brings essential health services directly to those who may face barriers to accessing conventional care. 

All services are provided free of charge, ensuring that financial constraints do not limit access to essential care. The program operates with strict respect for privacy and confidentiality, with all interactions conducted in accordance with professional and ethical standards. 

The program offers a range of primary and allied health services, including nursing care, dental support, physiotherapy, chiropractic care, remedial massage, and general pharmaceutical advice. These services are delivered by qualified professionals and trained volunteers, ensuring safe, person-centred care. 

Where required, participants may be referred to bulk-billing general practitioners for ongoing medical assessment and treatment, supporting continuity of care within the broader healthcare system. 

The Reach Out Health Hub Program focuses on early intervention, health education, and practical support. It aims to reduce barriers to care while promoting dignity, trust, and improved community health outcomes.
